Breath Alcohol Testing Instructions

Ordering Breath Alcohol Tests Online Instructions

1 - Login to your Account to Order Alcohol Tests Online - Click Here to Login

2 - Go to "Order Alcohol Tests" on the Top Tabs

  1. Select the Reason for Testing
  2. Find a Collection Site
  3. Order the DOT Breath Alcohol Test

3 - Check your Email for the QPassport

  • Quest Diagnostics emails the QPassport within 1 hour from "noreply@questdiagnostics.com"
  • QPassports are only Valid for the Collection Site that was Selected and Listed on the QPassport
  • You are only charged for Results not Orders

4 - Call the Collection Site to Confirm they are Open

  • Notify Drivers after you have received the QPassport and have Called the Collection Site to Confirm the Site is Open and Performing DOT Breath Alcohol Testing.  Some Sites may have Temporarily Closed or have Reduced Hours due to Covid, Staffing or Weather Issues or may require an Appointment

5 - QPassport Form

  • Drivers must Present a Copy of the QPassport Form with Proper ID to the Site Selected or the Collector can Scan the Barcode from the Driver's Cell Phone

  • If you did not Order Online: Members are Required to Pay the Collection Site Directly for the Test, and send Express Labs a Clear Copy of the DOT Breath Alcohol Results with the Receipt of Payment to Results@ExpressLabs.com
  • Invoices: Due Upon Receipt on the 1st of the Month for the Previous Month's Results and Members are Required to keep a Valid Credit Card in their Express Labs Account and Authorize Express Labs to charge for Drug & Alcohol Results

What is the Process for DOT Breath Alcohol Testing?

Members are Required to Order DOT Breath Alcohol Tests Online in your Express Labs account or Choose any Site that performs DOT Breath Alcohol Testing, Pay for the Test, and send Express Labs the Results and a Copy of the Receipt of Payment to Results@ExpressLabs.com

  1. QPassport Form: The Driver must present a copy of the QPassport form with Proper ID to the Collector at the site selected or the collector can scan the barcode from the driver's cell phone
  2. Check-in: Upon arrival at the collection site, the driver provides the QPassport and Identification and completes necessary paperwork
  3. Breath Alcohol Technician (BAT): A trained Breath Alcohol Technician administers the test.  The BAT will explain the testing procedure and answer any questions the driver may have
  4. Testing Procedure: The driver is required to provide a sufficient breath sample into an approved Evidential Breath Testing device (EBT).  The EBT analyzes the breath sample to determine the alcohol concentration in the individual's system
  5. Results: The Evidential Breath Testing device generates a printout of the test results.  If the alcohol concentration is below the DOT threshold (0.02), the result is considered negative.  If the concentration is at or above the threshold, DOT requires a Confirmation Test.  A driver with an alcohol concentration of 0.02 or greater, but less than 0.04, must be removed from duty for 24 hours
  6. Positive Results: If a driver's test result is positive (.04 or greater), FMCSA regulation prohibits a driver with an alcohol concentration of .04 or greater must be removed from the road immediately and given an SAP Referral.  The Employer to report the Positive Breath Alcohol Results to the FMCSA Clearinghouse
  • Calibration and Accuracy: The EBT must be regularly calibrated and maintained to ensure accurate and reliable results.  The testing process is designed to ensure the accuracy and integrity of the results.  The DOT regulations provide detailed guidance on procedures, training requirements for BATs, and consequences for non-compliance
