Breath Alcohol Testing Instructions

Members are Required to Order Breath Alcohol Tests Online (QPassports) in your Express Labs Account or Choose Any Site, Pay for the Test and Send Express Labs the Results.

Option 1: Ordering Breath Alcohol Tests Online

  1. Login to your Express Labs account

  2. Click “Order Alcohol Tests” on the Top Left Tabs

  3. Select the Reason for Testing (Random, Post-Accident)

  4. Choose a Collection Site by entering the ZIP Code

  5. Click “Complete Order”

  6. Check your Email for the QPassport from Quest Diagnostics within 1 Hour or download the QPassport Instantly on ESP

  7. Driver must bring the QPassport (printed or on your phone) to the Collection Site Selected on the QPassport for the Collector to Scan the Barcode and Start the Collection

  8. Call the Site to Confirm they are Open after you have received the QPassport before Notifying Drivers or Driving to the Site

  9. Breath Alcohol Results are Posted to your Express Labs Account and Uploaded to the Employer Solutions Portal (ESP)

Option 2: Choose Any Site, Pay for the Test & Send Express Labs the Results

  1. Find Collection Sites (No Restrictions): Google "DOT Breath Alcohol Testing Sites Near Me" Including: Concentra, Urgent Cares, Hospitals or Police Stations (If no other site is available) or use the Site Locater - Click Here

  2. Pay the Collection Site for the Test Results (Make sure you get a Copy of the Results)

  3. Call the Site to Confirm they are Open before Notifying Drivers or Driving to the Site

  4. Results: If you did not Order Online - Send Breath Alcohol Results with Receipt of Payment to Results@ExpressLabs.com (You will receive an Email Confirmation)

Breath Alcohol Testing Fees

  • $80 for Breath Alcohol Test Results when Ordering Online

  • Membership Fees do not include Alcohol Testing charges

  • Invoices are Due Upon Receipt - Members must keep a Valid Credit Card on File to Maintain Active Status for both the Company and Drivers
