Return-to-Duty Instructions

Employers Must Send Drivers for Return-to-Duty DOT Drug Testing (Observed Collection) only after Completing:

  1. Full Query in the FMCSA Clearinghouse
  2. Drug & Alcohol Policy
  • DOT Requires All Supervisors to Complete the Supervisor Training Course for Reasonable Suspicion Testing Before Hiring Drivers
  • Employers Must Report Negative Return-to-Duty Results and the Date of the Final Follow-Up Results to the FMCSA Clearinghouse

When are Drivers Adding to the Random Pool?

  • Drivers are Automatically Added to the Random Pool when Express Labs MRO Reports Negative Return-to-Duty DOT Drug Test Results
  • For New Hires, the Return-to-Duty Test Replaces the Pre-Employment Drug Test

Step 1: SAP Evaluation (Substance Abuse Professional)

  • The SAP Evaluation Determines:
    • When a Driver may be sent for Return-to-Duty Testing (Observed Collection)
    • How many Follow-Up DOT Drug Tests (Observed Collection) are Required

Step 2: Clearinghouse Registration & Full Query - Click Here

  • Employers must Conduct a Full Query in the FMCSA Clearinghouse to Confirm the Driver is Cleared for Return-to-Duty Testing
  • Drivers must provide Electronic Consent in the Clearinghouse
  • Both Employers and Drivers must be Registered in the Clearinghouse

Step 3: Drug & Alcohol Policy - Click Here

  • Employers must provide the Driver with a Drug & Alcohol Policy outlining Testing Requirements and Consequences for Violations
  • Drivers must receive a Full Copy of the Signed and Dated Drug & Alcohol Policy

Members are Required to order Drug Testing Online or by using Paper Quest Forms.  After Enrolling, Express Labs does not accept Drug Test Results from other Companies.

  • Option 1: Order Drug Tests Online (QPassports) in your Express Labs Account
  • Option 2: If you cannot order online, use the Paper Quest Forms provided by Express Labs
  1. Login to your Express Labs Account.
  2. Click Order Drug Tests in the Top Left Tabs
  3. Select the Reason for Testing (Return-to-Duty or Follow-Up)
  4. Choose a Collection Site by entering the ZIP Code
  5. Click Complete Order
  • QPassports are Emailed from Quest within 1 Hour (noreply@questdiagnostics.com)
  • Call the Collection Site to:
    • Confirm They Are Open
    • Confirm a Same-Gender Collector Is Available for Observed Collections
  • Drivers must bring the QPassport (printed or on phone) to the Site Selected for the Collector to Scan the QPassport Barcode
  • DOT Requires Drivers to Get Tested Immediately when Notified for Follow-Up Testing
  • Note: Collection Sites may not begin a DOT Collection within 3 hours of closing (Drivers are allowed up to 3 hours to provide a urine sample)

Step 5: Report Return-to Duty Results - Click Here

  • Employers must Report Negative Return-to Duty Results to the Clearinghouse

Step 6: Report Final Follow-Up Results Click Here

  • Employers must Report the Date of the Final Follow-Up Test to the Clearinghouse

Follow-Up Testing Instructions

  • Employers Are Responsible for Scheduling and Notifying Drivers for Follow-Up Testing based on the SAP Evaluation
  • DOT Requires a Minimum of 6 Follow-Up Tests in the First Year, with Testing continuing for up to 5 Years if Required by the SAP
  • DOT Requires Drivers to Get Tested Immediately when Notified for Follow-Up Testing

What does "Get Tested Immediately" mean?

  • Get Tested Immediately means once a Driver is Notified for Follow-Up Testing All Actions must lead Directly to the Collection Site to get Tested
  • Notified means a Driver Knows they were Selected for Follow-Up Testing
  • Refusal-to-Test means Drivers who do not Get Tested Immediately after being Notified for Follow-Up Testing is considered a Refusal-to-Test, which is the same as a Positive Result

Supervisor Training Course (Online)

  • DOT Requires All Supervisors take the Supervisor Training Course for Reasonable Suspicion Testing to Hire Drivers (Including Owner Operators who hire Drivers).  The Online Course can be ordered on the Left Side Tabs in your Account

Drug Test Results

  • Drug Test Results are Emailed Directly from Express Labs Medical Review Officer (MRO) from email@drsmro.com
  • Results are also uploaded into your Express Labs Account
  • Expected Timeline: Most Negative Results are Reported within 3 Business Days from the Date the Lab receives the Sample
  • If Results Are Delayed: If you have Not Received Results within 4 Business Days, You may Contact Quest Customer Service at 800-877-7484
  • Provide either your Quest Account Number (Found on the QPassport) or the Specimen ID (Found on the Paper Quest Form) to assist in locating your Results

Express Labs Testing Fees

  • DOT & Non-DOT Drug Test Results (In-Network Collection Sites): $80
  • Collection Fees are Included at In-Network Sites but not at Out-of-Network Sites
  • Return-to-Duty & Follow-Up Drug Tests (Observed): $125
  • Breath Alcohol Test Results (Online Orders): $80
  • Billing & Payment: Invoices are Due Upon Receipt - Members must keep a Valid Credit Card on File to Maintain Active Status for both the Company and Drivers

What is an Observed Collection?

  • The Collector (Same Gender as Driver) accompanies the Driver into the restroom.  The Collector must directly watch the urine stream go from the Driver’s body into the Collection Container.  The Driver must raise their shirt above the navel and lower clothing to mid-thigh to ensure no devices are used to cheat the test.  The Collector signs Documentation confirming the process was completed according to DOT guidelines

What if an Owner Operator Test Positive?

  • Express Labs does not Manage the Return-to-Duty/Follow-Up Process for Owner Operators.  Owner Operators who have a Violation in the FMCSA Clearinghouse can not Enroll in Express Labs Consortium.  Owner Operators who Test Positive for Drugs or Alcohol or Refuse-to-Test will be Terminated from Express Labs Consortium

Drug Testing Requirements

Members are Required to Order Drug Tests Online (QPassports) in their Express Labs Account, or use the Paper Quest Forms provided by Express Labs if they cannot Order Online.

  • Upon Enrolling, Members are assigned a Quest Diagnostics Sub-Account Number, which must be used for All DOT Drug Testing.  After Enrolling, Express Labs does not accept Drug Test Results from other Companies.
  • Drivers can only be Added to the Random Pool when Negative Pre-Employment (or RTD if the Driver has a Violation) DOT Drug Test Results are Reported by Express Labs Medical Review Officer (MRO).  Drivers cannot be Added with Drug Test Results from another Company
  • Random Drug Test Selections can only be marked Complete when Negative Random DOT Drug Test Results are Reported by Express Labs Medical Review Officer (MRO).  Random Tests cannot be marked Completed with Drug Test Results from another Company
  • Drug Test Results: DOT Requires Express Labs and the Employer’s Company Name to be Listed on All DOT Drug Test Results, which are Posted to the MIS Report that All Members need for DOT Audits.
